Login into your gmail account, go to Google docs directly from this link (Google doc) or from your gmail inbox area, click on apps button on top right corner and select drive. In Google drive click on create button top from left corner and select form. The easy form builder will be in front of you. Type multiple questions in it and choose the answer type. When you complete all question, click on send form button. Select how you like to use this form either send into your social network or embed into your website.

How to use Google form into you website.

When you completely created form from form builder, click on send form button. Here you will see single line code. Copy that code and paste into your social network timeline like facebook, twitter etc. If you would like this form to appear on your website page. Click on the embed button beside the single line code. The embed code will be shown to you. Copy that code and paste it into any where into your page. In WordPress you can create a particular page for this questioner or survey, also you can paste this code to you widget area.

How to protect multiple entries from a single visitor?

Of course this very important that what data you are required from public or private should not have duplicate entry.  It must be protected, if someone fill up your form multiple time, it will be difficult to analyze. To avoid this on top of this form you will see a 3 check boxes, among them click on “Only allow one response per person” and also you can choose other to option as per your requirements.

How to get survey results.

Next, after a certain time period you must required survey results for which you have created this form. On the same form’s screen in Google drive, you can find view responses on top, click on it, it will create a spread sheet for the responses. The all results will be stored in this spread sheet. You can check results by making single entry by yourself.
